277 INTRODUCTION Pulmonary hypertension is a progressive disease that ultimately leads to right-ventricular failure and death. It is characterized by an elevated pulmonary artery pressure and pulmonary vascular resistance. The incidence of primary pulmonary hypertension (PPH) is estimated to be 1–2/ 1000 000/year and approximately 6% appear to be familial. There is a preponderance of females among PPH patients with a ratio of female to male varying between 1.7 and 3.5:1. DEFINITION Pulmonary hypertension is defined as an elevated mean pulmonary artery pressure of 25 mmHg or greater at rest or 30 mmHg with exercise. In 1998, the World Health Organization set out new guidelines for the re-classification of pulmonary hypertension (Boxes 19.1 & 19.2).
